One good idea is to water down a bit of frosting and use it as 
a 'crumb coat' first.  It should be about the consistency of pancake 
batter and it seals in the crumbs.  Sticking the crumb-coated cake 
in the freezer for a few minutes before the final frosting also 
helps.

The best way I've found to frost smoothly is to use an offset 
spatula and put the cake on a turntable.
