Hi Edmundo, 1. About the english text in the form. This is due to the fact that there are text strings in the language files that are not still translated in brazilian portuguese (pt-br). The language files for pt-br are located in the /language/pt-br directory. You may search for the corresponding string in all files in the above directory and translate them. It is a good idea to check all files in the /language/pt-br directory for untranslated parts. To save you some time, the file containing the string for this form of the Radiology module is lang_pt-br_konsil.php.
2. About the lab address. It appears that there is no graphic interface to change it. You have to edit a configuration file: /global_conf/inc_global_address.php. 3. If you want to change the layout of the form then edit the /modules/nursing/nursing-station-patientdaten-doconsil-radio.php file. The request form starts from line 339. Unfortunately, it seems that this part is not separated in template. Hope this helps. If anything is unclear, just ask.
