Hello jENELLE,

All services, including the laboratory tests, will be automatically
allocated by the billing system. The billing officer gets a list of items
what must be charged for a patient. He, the billing officer can now decide
if a drug is paid by the patient and he can mark it (he can also say that
this drug is not paid now). If it's paid, the pharmacist can see that he can
give it to the patient afterwards. This also includes the circumstance that
a patient could also be discharged till the e.g. laboratory results are
available. Also there is a button for extremely poor patients; these costs
will be allocated to the account of a kind of insurance (what will be
covered by the hospitals or other funds).

I think I understand what you describe in your email. We want to avoid that
on a billing module there must be the items charged by the hospital again.
It's still in the database, why not using these kind of information. At the
beginning there was just drugs what must be charged, after that also xray,
registration fees and other service based positions are also added to this
table and the table name was redefined from drugs to drugsandservices. There
are just the laboratory tests what are located to the table where all these
tests will be stored for hospitals. Reason was not loosing the route back to
the main branch; the laboratory modification of us should just be an add-on
and not a complete replacement.

The existing ecombill is worse and in my eyes not really working or smart.
It is too simple that here all the benefit of a computerized system can be
used. The other scope is that we have a system what has also the way of cost
control.

The system we are follow is that there is just one server at the hospital.
Here there are all backup routines stored, UPS and other kind of
technologies like RAID what makes it safe. The clients are completely
platform independent, they need just a web browser and an network connect.
This is just the main idea of care2x.

Hi Robert,

I understand what you mean.

I just reviewed the branch and I have another question in mind. As the
patient goes through laboratory, diagnostics, surgeries, etc., are the items
automatically billed or does the billing department have to manually add the
items to the final bill?

We're currently using a Windoze based system (which we aim to replace
because of costly licenses involved) and it does this. Were looking for the
same functionality, however. The billing department only has to double check
the items in the bill, they dont have to add items one by one.

Based on your email, since all the items and drugs are in one table, is it
correct to assume that for every service/drug/item available in the care2x
trunk, you would have to create another item of the same description ( i.e.
x-ray in trunk, another x-ray under the billing table for the pricing)?

We were thinking of adding a column (maybe pricing?) to appropriate tables
already present in the trunk and create a billing module which would pullout
billing data for a patient based on the test/services/room rates and present
a bill automatically? Any suggestions?

Thanks,

jENELLE

Hi JENELLE,

If you can check webERP also for your requirements it would be also helpful
for us as well. Then we have bigger, more widespread requirements and maybe
you will find some hints what we do not see for the moment.

If webERP is the way we can go, next would be an interface between care2x
and webERP.

For the moment we do not have valid documentations, the requirements changed
too fast and we had no time to update our work quite well. But this is also
on the top 10 of the to-do list. Basically to install the billing module to
the main branch it is possible. All price information are stored in a table
called drugsandservices. On this table we have all price information; all
other information like demographic data of the patient or the laboratory
tests will be picket out on the existing tables. Maybe some little changes
you have to do, but not as much as you think.

Hope that helps

 Robert

Hi Robert,

Thanks for the prompt response.

Yes it seems that there are workflow similarities between here and Africa.

We will check on WebERP and provide feedback.

We can develop modules/plugins which will match our workflow here but we are
trying to minimize the amount of work to be done so that it can be
implemented ASAP. We we're glad to see that your Billing module has parts of
what we wish to develop and will help us reduce our workload.

Another question, would the billing modules on this branch work with the
current release of the trunk? We havent checked on the changes you have made
from the trunk yet. Would you happen to have documents to help us out
regarding changes to the db, classes, etc?

Thanks again,

jENELLE

Hi jENELLE,

What we did so far is to cover all these processes of a typically outpatient
clinic in East Africa to care2x. We did not touch the inpatient workflow so
far, we are now starting with it. I can imagine that the "workflow" of the
work in your hospital is quite the same like in East Africa. Simple process
changes are e.g. that a patient mostly does not have demographic information
like street, but district. They can be easily identified by father's name,
name of the tribe or a contact person in town.

The outpatient process follows now the workflow of an patient from
registration to all outpatient departments till he gets the invoice of
services and drugs, that e.g. the pharmacist can see in a list that this
patient has paid for this drug and he can hand it out.

Let's stay on this example. Inpatient workflow is now different: A ward will
get a kind of sub-stock, some drugs and medical items ordered by main stock
for the patient. But e.g. no pain killers, these must be ordered different.
Now there must be a follow track what is handed out to the patient and will
be invoiced. These covered up some other ideas what makes more sense to
focus now the main work of the pharmacist at the main stock. Here we have
purchasing prices from different dealers and also the price to the patient
could be different. Now we got the idea to implement here a compatible ERP
system to manage stock. This ERP system could also manage the typically
accounting of the hospital itself. Main focus is now to find an ERP system
what could be compatible to Care2x to split off the workflows of accounting
and HIS. We are now looking to the idea of webERP if that could cover these
processing.

Hi Robert,

I just checked out the branch you mentioned and I have some questions
because we are looking to implement Care2x here in the Philippines.

Does the branch also have prices for rooms/wards? How about outpatients who
would only want to purchase from the hospital's pharmacy but are not really
seeing any doctor or are not having something checked, something like a
pharmacy POS, is it addressed in the branch?

Thanks,

jENELLE

Hello Jayantha,

We realized a new module to do that. You can look to the EastAfrican
version, on the laboratory module we changed following:

==> Test parameters are based on the database

o Each hospital can maintain own LabTests what they could provide
(or not)

o We added also these parameters you described (upper, lower .)

o If you want more parameters, now you can add it dynamically

o And also a price charged to this patient (requirement of
EastAfrican hospitals, self pay patients)

It's working fine. but for the next generation I would suggest:

==> Please do not follow the idea to store each test parameter and/or
result to one row on a table cell (by using serialized strings).

You can find our work at sourceforge., branch is elc_tanzania-0-2

Hi,

In the current version of care2x, there is no facility to enter test
parameters (upper boundary, lower boundary, etc) separately for male and
female.

Could somebody tell me how you are all managing? Any advise on how to tackle
(work around) will be really helpful.

Regards
