We have level 2 transparency in the .NET 4 builds, not enabled yet by default though. Regardless of security transparency level it's my understanding you still need the APTCA attribute to be able to run in the ASP.NET medium trust sandbox. Or is [SecuritySafeCritical] completely the same?
