Dear All, I want to collectively thank all colleagues who submitted - and hopefully will continue to submit - material in response to my inquiries.
I often get so much - thank you - that I cannot possibly use everything that is in principle useable, and I have to make a selection. So I will thank all that have contributed anything - irrespective of whether the material was used or not - in an introductory acknowledgement section, and provide additional specific acknowledgements in the places where I actually do use the images. I hope this is acceptable, please keep sending stuff when I inquire. Also, please inform me if there are any copyright issues to resolve, e.g., an image has been published before.
