The first time your client boots OOo the license is displayed and they have to sign off on it. No physical paperwork is required although something with the standard scary "Stability Disclaimer" added to your own Warranty would be wise and in some jurisdictions, required. The OOo Art project page (http://marketing.openoffice.org/art) has Art work that you can add to to docmentation or website and "OpenOffice.org inside" type labels Because OOo is open source, customising a version tailored for your business is also an option.
