3/57/0 stands for "UNABLE TO RECOVER TABLE-OF-CONTENTS" (see
http://fy.chalmers.se/~appro/linux/DVD+RW/keys.txt for transcripts),
which suggests that the media is all screwed up. What media is it
anyway?

You don't need to re-format DVD+RW media, just write over the old
recording with growisofs -Z command. It's actually recommended, as it
was observed that excessive re-formats may render media unaccessible.
Yes, one can argue that 3-4 times is not excessive number, but on the
other hand it's a firmware bug (I actually could re-format "toasted"
media in a unit of another brand) and it's hardly arguable. I mean bugs
seldom, if ever, make sense.

But in either case, the cause of the problem might be something else. At
least I've just obtained a problem report from another DVR-x06 user, who
claims that DVD+RW recording performance is lower than expected and that
unit report "No medium found" at "stopping de-icing/writing lead-out" at
program exit.  Can you confirm any of this? The performance problem is
already resolved and the second one is being looked into. As soon it's
resolved, an updated version will be posted. It (the second problem)
might as well be a cause of your "unable to recover table-of-contents"
problem...
