Chad  is right Erwin is an excellent top of the line tool. It is also 
pretty expensive (thousands of dollars)  For the low budget individual 
developer , Dezine is a good logical data modeling tool that exports, 
Access, Sql Server, Oracle, and many other database output scripts. It also 
has quite a number of pre-defined out reports for hard-copy data 
dictionaries.  For  $139 it is a good value.  They also sell import modules 
that you can import an access (or SQL, Oracle) and it will build you 
logical model diagram.
