We use Packet8's Virtual Office in my office space and run it over 
Adelphia Cable service at their lowest bandwidth package (3M Down, 256k 
up).  This works fine most of the time for the 5 VoIP lines we have 
though about a week ago someone left a P2P app running and it killed the 
connection to the service.

We went from paying $180/mo for 3 copper pairs with no long distance and 
no features to paying $200/mo for 5 VoIP lines with full PBX feature 
sets.  We did keep one copper pair for faxing as Packet8 doesn't support 
faxing yet.
