Actually, looking back on it, I think mine was just too big for my mouth, it tended to roll over when I was eating (sideways), then I'd crunch down on it sometimes. Was eating a bagel one morning at work, and bit down too hard. That really sucked, more a chip than a crack, tho.
I took it out right then and the hole had closed up by the next day. S'OK, though. I always felt like that thing was a festering bacteria laden thing that gave me bad breath, no matter how many times a day I cleaned it.
