Michael,

I worked at Kennedy Space Center building web
applications, and we had to provide a System
Requirements Specification, which describes the system
and software requirements for applications.  

While I can't say if it exactly follows what the
government requires for a Preliminary Design
Requirements report, I'm sure it's close.  Let me know
if you want a copy.
