Just to play devil's advocate: it could also be interpreted by your
version that your project management was horrid.  If your requirements
are set and written into the contract, then your customer should have
no reasonable basis for complaint.  Of course that doesn't mean they
won't, but your PM should steer the scope creep into a "phase 2"
project.

And it may not have been the PM - could've been the BA/SAs failing to
properly define the requirements.

I guess my point is, if you point the fingers at yourselves that's
usually where the fault lies.
