Two points on this:

1) Are you talking about a code error in software?  Are you in QA?
This sounds like a QA process problem not a single person's error.

2) If "the management" is so disconnected that you have to fill out a
form just to let them know what's going on with their own clients then
that's in itself could be an indication of a pretty serious problem.
