Have you taken it to the vet yet?
It was a stray so you never know and this problem can either psychological or 
internal, since you say it does go in the box sometimes. I would take the cat 
in with a stool sample and first check the stool for parasites and if that is 
ok do a full panel blood workup because something is probably going on there.

You might also try a different cat litter, I have seen cats just not like one 
brand over another before, this was especially true years ago when "clumping" 
litter came out.
