You would be looking for an ATI Wonder series of cards.

These aren't the best currently, by any stretch , for gaming.

But they were the best mix between 3D performance and video input/outputs.
They come with a seperate breakout box that has just about every type
of output/input you could want back in the day.
I don't know if they have been updated to HDMI, but due to the
draconian Protected Path requirements placed on manufacturers I
wouldn't be surprised if they had not been.
