The other possible issue could be grounding, either inside the computer or
on the circuit.  Is this an issue that has just started happening, is it
something that has gotten worse with time or has it been going on all along?

One thing that I have found is that if my speaker volume is cranked and my
PC volume is turned up as well I "hear" the hard drive when it is accessing.
Is this something similar to what you're experiencing?  I'm not sure what
the "cure" for it is unfortunately... I just don't crank my speakers all the
time.
