I got to admit, the false receipts thing needs to be looked into.  Frankly,
I expect the results to be more incompetence versus receipts.  Grunts are
prone to see things in the light that their bosses want to see them.

There is evidence of it. For instance Powell referred to a set of aluminium
tubes during his UN speech and presented a manifest for them that they
claimed showed Iraqi attempts to create nuclear weapons. It came out a
couple of weeks later that the evidence was forged. According to the
Washington Post, while Powell did not know of the forgeries, several
defense department and White House staff did.
