I got it off a yahoo search when I was searching for University of Phoenix
Online. Originally I was on a CFUG site when I saw a link to the original
request form and I found it interesting for getting a masters degree in CIS.
They have a number of different sites that all point back to the same
request form. I happened to stumble onto this link on the third or fourth
page of the listing. They also had a bunch of affiliate sites advertising,
so I had to search a little.
