Yes.  It does so by calling stat() on the file -- a very fast operation.
You should not have to worry about this at all.  The only time you really
have to worry is if you actually see your swap space in use -- a situation
generally remedied by adding more ram.

The rest of the time, Linux will just do its thing without any need for help
from you.  I have more than one Linux machine here which has not been
rebooted in more than a year.
