I'm having the exact same issue and have spent quite a bit of time trying to figure it out.
I am on Windows 2003, J2EE EAR installation, JRE 1.4.2, with CF7 running on the same box. I think my environment has something do with it, but can't figure out what. I have "Enable Flash Remoting support" turned on under Flex Intergration in CF Admin (have you checked this, Bert?) If I click the Launch Server Monitor from CF admin, I get a blank, blue-gray screen. The SWF is loaded, but doing nothing. If I try to hit the SWF directly using the IP address (http://localhost:8199/CFIDE/administrator/monitor/ServerMonitorUI.swf) I get a HTTP 403 Forbidden error. If I right-click on the SWF file in Windows Explorer and open with Internet Explorer, it opens and I can type in my server IP, port, and login, but comes back with "ColdFusion Server is unavailable." Multiserver monitor behaves about the same way.
