I'm having the exact same issue and have spent quite a bit of time trying to 
figure it out. 

I am on Windows 2003, J2EE EAR installation, JRE 1.4.2, with CF7 running on the 
same box. I 

think my environment has something do with it, but can't figure out what. I 
have "Enable 

Flash Remoting support" turned on under Flex Intergration in CF Admin (have you 
checked 

this, Bert?)

If I click the Launch Server Monitor from CF admin, I get a blank, blue-gray 
screen. The SWF 

is loaded, but doing nothing.

If I try to hit the SWF directly using the IP address 

(http://localhost:8199/CFIDE/administrator/monitor/ServerMonitorUI.swf) I get a 
HTTP 403 

Forbidden error.

If I right-click on the SWF file in Windows Explorer and open with Internet 
Explorer, it 

opens and I can type in my server IP, port, and login, but comes back with 
"ColdFusion 

Server is unavailable."

Multiserver monitor behaves about the same way.

Any ideas would be very much appreciated!
