There are third party "plug-ins" for Acrobat 4.0 that can do this. An
example would be the electronic version on CD-ROM of the Mastering
ColdFusion 4.0 book by Armin Danesh. Have a look at the Acrobat site
(http://www.adobe.com/acrobat/) or the PDF Zone at
(http://www.pdfzone.com/products/software/index.html).
