You say:  "Its a false distinction that has a definition only within the minds of 
those who want to make the distinction."  

Is it not you who is making the distinction?  I personally don't care what someone 
calls themselves, until it comes to hiring.  If someone comes in to my company calling 
themselves a programmer, I will grill them on Software Engineering principles.  Its 
the same thing with people calling themselves "Engineers" a few years back.  Everyone 
was an Engineer, but a **TRUE** Engineer comes from a much more strict background of 
disciplines, and a better mindset in solving Engineering problems.

That said, I will not hire someone who touts being a programmer if all they list as 
their expertise is ASP, JSP, CF, PHP.  I look at their schooling as well, to see if 
they have a CS degree...something that instills good PROGRAMMING principles, not just 
the ability to program.
