If you're dedicated then think about Cybercon.com. I'm on my 3rd year there and have been very happy. They don't provide managed service though (or at least not to me with the prices I'm paying), or software, so you have to bring your own licenses or buy/rent from them. They do ping the box every 3 mins and reboot it if it goes down, and I can always call them 24x7 and get a real person if I have an emergency. Neither is a common occurrence but of course it can happen.
One neat thing is you get 32 IPs with each box; plus you can buy more dirt cheap.
