I do it manually, using Adobe PDF writer (it has functions to create
binders, where you can put in multiple documents and it makes them all into
one pdf).  

I'm sure there is a programmatic way to do it, but not sure if you can do it
using the writer.  I would probably look into doing it with ghostscript
and/or some commercial products out there (I remember there being one to
create pdfs from CF, but it was pretty expensive).
