I think PDF is much more common for this kind of thing.

Flash adoption is almost certainly higher. Unfortunately, I haven't found a
good source for PDF availability statistics.

That's correct. However, it's just a SWF, so a user can save it like they
would any other SWF.

I would recommend PDF for forms. It has lots of potentially useful options -
automatic submission via HTTP or email, digital signatures, etc.

Actually, I suspect that FlashPaper will be around for a while at least.
It's quite useful in some niches where PDF is not (yet), and it's not a
serious competitor to PDF outside of those niches.
