Another vote for Unfuddle here. I was going to recommend it, but I was beaten to the punch. Works great, nice ticketing, bug tracking, notes, etc features as well. It's not expensive either. Also, given that it's hosted, you have offsite backups. Used in conjunction with TortoiseSVN, it makes for a nice, smooth system.
