Having been a consultant for 20+ years (really) here is my 2 cents:

- Many years ago, only the 'brave' went for consulting. Then other people heard about 
tha brave's hourly rate, they
muliplied it by 2000 and said: 'I wanna be a consultant'

- You know about the 'hidden' costs:
    You market yourself, count the hours/expenses as part of your hourly rate (you 
spend 100 hours to get a 100
hours job... you figure)
    Sick leave, vacations, buy your own books, study late night, participate on 
mailing lists, etc, etc...

- Big corporations rather pay $150 to an agency, of which you may get $100 (you have 
to run your C-corporation,
having a $1million insurance, etc...) so the agency would have to deal with IRS, etc...

- Companies (big or small) rather spend some extra (or seemingly extra) money to get 
someone to "get the job done"
and they think, it's going to be "THE" contractor. In some cases it works. Most of the 
time (these days) it
doesn't, so they hire another consultant.

- Finally, the actual rate can be VERY flexible. If they need you, they will pay you 
almost ANYTHING...

One final thought: I have the best references from all my clients, and that always 
helped me.
