I'm glad you didn't ask that; there isn't really a good answer. It's a PITA, as you mentioned. As a consultant, I mitigate it by remembering that I charge hourly rates. It used to be that this problem didn't come up that much. People who were willing to load test were also willing to run dedicated servers for their mission-critical apps. However, now that the price of load testing is decreasing, and the price of clustering is also decreasing, I'm seeing more places distribute all of their apps on each server, and simply clustering those servers, which does make testing in general (load testing and troubleshooting) more difficult.
