But, if you are uploading to the same filename, you could attempt to see if using a random filename and storing that with the product would actually work.  That should force any caching problems to be highlighted (if it loads the new file).

If nothing else, if the page (on refresh) still shows you the OLD filename first - then it IS a caching problem somewhere (and it may be out of your control if it is a dodgy cache somewhere in the route to the website).  Note that some caches will only change a file once a day or once every or if the size is different.  If they are just changing colours on the image (or something similar) then the size of the image may not change.

If the filename itself changes and has the OLD content then that is REALLY bizarre.
