RT (http://www.bestpractical.com/rt) is pretty good. Fairly deep and complex initially to set up, but worth the time. It's free, too.
Good things to note: * APIs for web, email, command line * OO Perl * Runs on just about anything *nix, but Windows ports available * MySQL back end * Can be integrated with Bugzilla and/or CVS with a little elbow grease * Insanely granular security * Good i18n * Not too scary for end users (we scared some of them with Bugzilla) Happy to give you a hand if you need one.
