I quite like ArgoUML, but the UI *is* rather ugly.
Poseidon (http://www.gentleware.com/) is based on ArgoUML, and has a much
nicer UI (seems to be more stable, too) - I think you need to buy the full
version to generate anything other than Java stub code, though.

Theres also an ActionScript generator for ArgoUML, for all you Flash freaks
(thanks to Chris V for the link):

But when all else fails (ie. I'm on the crapper with no laptop) I bust out
the trusty notepad & pencil :)
