First and foremost - call your vet. If you don't have one, make 
arrangements to get one. The vet can tell you when to bring your 
puppy in.
Shots and worming - I think usually around 6-8 weeks.Weaning is 
usually 8 weeks and is done by introducing very small amounts of 
puppy food once a day. Then gradually increasing the feedings to at 
least 4 times a day.
As far as puppy food, I'm not a fan of canned food. It causes the 
bowel movements to be soft, dark, smell bad and attract flies. Dry 
dog food does not. The bowel movements are firm (easy to pick up 
accidents), don't smell (no odors from accidents in the house) and 
light colored (easy to notice blood or worms). I fed my Chi Pedigree 
dry puppy chow. I put the kibble in the blender and ground it down to 
a course powder. Then added enough warm water to make a paste (like 
oatmeal consistency). My vet said that as long as the protein content 
in the puppy chow was good, I didn't have to buy the expensive food. 
Whichever type food you give, make sure it's in very small amounts at 
first. Maybe a teaspoon. The puppy's little stomach needs to "learn" 
to digest new food. You need to watch for vomiting and diarrhea. If 
they happen, stop the puppy food for a few days and try again. If he 
still gets sick, switch the type/brand of food.
PS Why is the mom getting replacement milk? That's for puppies too 
young to wean.
