In actuality, the ADA is federal law, not state....so it is the same in every state. The ADA DOES give the right to inguire IF one has a disability. They are not permitted to inquire as to the nature of the disability, but inquiring if one has a disability is permissible in the area of service animals. You may be confusing the law aqainst inquiring about a disability for other purposes....for instance, if you are trying to get an apartment or something, the may NOT ask if you have a disability...but if you are accompanied by a SD, they ARE allowed to ask if you have a disability...they just are not allowed to ask what the disability is, ect.....
The ADA does not make a distinction between any type of service dog. IT DOES make a distinction between therapy dogs and service dogs. Therapy dogs do not have the same access rights as service dogs....but they do have added protections over pets. For instance, a therapy dog is protected in housing and can live in a place where pets are not permitted, ect.....but under the ADA, they do not have access rights to most public places. HOWEVER, many, if not most, states do have added protections that give therapy dogs access rights...but those rights are state rights not federal and can vary from state to state. The rights of Service dogs are federal and can not be taken away by states.
