I'm currently doing exactly what Aaron describes when a user clicks my 
toolstrip button to figure out if this is the first time the extension 
has been used.  I also store our version number in local storage so I 
can tell if the extension was updated or not (I tell the user that an 
update occurred with a link to any new functionality).
