Extensions are pretty harmless. They're essentially GreaseMonkey scripts plus an API for the tabs. If you are concerned about security, you can see the source for any extension you install by looking through the Scripts section of the DOM Inspector for any page the extension works on.
I can certainly understand not wanting third party code mucking with your experience, but since it's all so transparent, you could find some simple little enhancements to fit your needs pretty easily. Hell, you could write your own. (I did).
