I had a quick look at your configuration and I don't see anything obviously wrong with it. A few questions:
I don't think this should be the case but you don't run Samba on the same box by any chance? Do you see any errors in the log when your access fails? Do you see any crash dump on the system? You might want to monitor smbd's PID to see whether the service restarts for some reason when the access fails.
