Just few quick questions.
I'm not an 'expert' on cisco routers.
I probably know less than you do.
But my questions are these
1. Is console cable going through any magnetic field or any strong elec.
equipment at all?
2. If cable is okay, what about your computer serial port connector or the
router console port connector? Are they clean?
3. Have you tried to use different terminal software/ different computer?
If all above are okay, perhaps it is the console port length limitation.
I will check in Cisco site and will get back to you if I find it.
Let me know if these helps (even though I'm thinking that you already
checked these already)

I have extended my console port with the CAT5 roll over cable and it
extended to 10 meters .... i worked for months on this cable and now
last month two of my router console port is hang out ... Console screen
is frozen but it is excepting data from console port so i blindly place
a password on VTY and have access the router from interface.
If someone can tell me what is the problem.
The terminal config is OK
the cable is OK
