You haven't installed the Wadalab fonts correctly which contain a
c42min.fd font definition file and proper font files.

Yes. The CJK package has been set up so that all examples (except the
one which uses cyberbit) are working out of the box. cyberbit.ttf
isn't part of TeXLive because this font isn't a free one.

I don't have MikTeX, so I can't comment on this.

Well, using CJKutf8.sty, this can't be simplified to

  \documentclass{article}

  \usepackage[T1]{fontenc}
  \usepackage[german]{babel}
  \usepackage{CJKutf8}
  \usepackage{ruby}

  \begin{document}

  \begin{CJK}{UTF8}{song}

  \tableofcontents

  \section{\ruby{寿司}{すし} besteht aus Reis und Fisch.}

  \ruby{寿司}{すし} besteht aus Reis und Fisch.
  Lecker ist z.\,B.: Thunfisch 鮪.

  \newpage

  \end{CJK}

  \end{document}

[The final \newpage command is necessary; otherwise the entry to the
 .toc table is written outside of the CJK environment which leads to
 an error.]

The Wadalab fonts have a special subfont layout (for example,
`dmjhira', `dmjki', etc., instead of the normal `dmj01', `dmj02',
etc.), which I have called `dnp' for historical reasons; cyberbit
doesn't have this.

Yes, there is no `dnp' subfont layout for utf8 font encoding.

Apparently, you haven't installed the cyberbit font correctly.

Installing CJK fonts have been discussed again and again on this list;
maybe you take some time to walk through the mailing list archive.
