I've no idea about what files you are talking about.  Please note that
I'm not a user of cct and its related packages, thus I can't help
here.

This is what you can find e.g. on TeXLive (but with other fonts, of
course).  Note that the right way is to

  . set-up a proper MAP file for dvips (usually in
    $TEXMF/fonts/map/dvips/...),
  . add an entry to updmap.cfg,
  . run `updmap'

Then a proper map file for pdftex is generated automatically (using
the PFB files).

Yes; the ENC files specify the used glyph name slots in the subfonts.

(e.g. pdftex-a.pdf), section `Map files'.
