I don't have that form on my computer.  It was actually typed a very, VERY
long time ago on an ancient typewriter.  I've been using the same form for
20 years!  I just keep making copies.

I will gladly send anyone a copy of it, along with my warranty, etc.  I have
already done so for many others, even sent one out very recently (Kathy?).

One of these days I will remember to type it.  Actually, it is not all that
much to type and I can't imagine why I haven't done it already.  More than
likely it is because, being the perfectionist that I am, I would attempt to
make it better and end up spending a whole day on it.
