You need to do something appropriate to sendmail.cf or the milter configuration (which I know nothing about I'm afraid) to do this.
This is not something that can be configured in clamav AFAICS.
He was referring to the clamav-milter, which *does* hook clamav into sendmail, and is included as part of the clamav package.
