That being the case, I think Exchange has an API for submitting messages to a virus scanner. If you can get Clamd to run on Win* (may or may not be very easy because I think Clamav assumes a POSIX environment) then the next step would be to write something to the Exchange API that takes the message, submits it to Clamd and then disposes of the message appropriatly.
