OK, on the create user page for ALL, under the content section, do you
happen to only have "Local DB" checked as the Available Providers?

Yes Dan, I did try utilizing the ALL to catch iOS devices (as others
suggested), but then I encountered the "Invalid provider name"after
entering my credentials.
I'm also currently seeing constant "Invalid authentication provider,
<Local DB>[xx:xx:xx:xx:xx:xx ## yyy.yyy.yyy.yyy] userID" in the event
logs, and every MAC OUI I've looked up is Apple.
Looks like they're all erroneously hitting the local DB instead of our
configured provider for AD.

Are you seeing the "Invalid provider name" on the OS user page or
after a login?

We recently upgraded to 4.8.2 to get support for OSX 10.7 (Lion), and
our MAC_ALL workaround for iOS stopped working as well. It appears as
though iOS/Mobile Safari is no longer getting identified as MAC_ALL
because I get "Login page is not configured..." If I setup an ALL
page I get a login prompt, but then am presented w/ "Invalid provider
name" error.
Has anyone seen this behavior or figured out how to get iOS working again?

Check under your User Pages to see if the option to "Use web client to
detect MAC address and Operating System." is selected. We user
separate user pages for different devices and the iOS devices fall
under the MAC_ALL classification for NAC.
