None of the logs looks definitively likely to be the cause for failing the voice call to start. I think the best way to progress this will be to reproduce with the log_level set to 5 (as described at https://github.com/Metaswitch/clearwater-docs/wiki/Troubleshooting-and-Recovery#sprout) and share the logs. Also, please can you make a note of the time that the call fails?
