When your key changes, Putty prompts you to update the key, continue without updating, or cancel. So, you would have known immediately if this was the cause.
Yep, upgrading SSH can do that (it did it to me when I applied a newer version of OpenSSH). You can also check the sshd config file (/etc/ssh/sshd_config on my box), for any restrictions. You can setup who is allowed to run an SSH session in there... Glad you figured it out though.
