cfengine might be a bit of overkill, but it has this functionality built in: http://www.cfengine.org/docs/cfengine-Reference.html#processes
Alternatively, we also use CheckService on some machines: http://www.linvision.com/checkservice/ but it only sends us notification when a service has died - I've got no clue if it can restart them.
