We don’t down stage except for the MDS and 1 in house monitoring form. Last skin seminar I went to said not to and I haven’t heard of any changes.

What do other facilities do as far as down-staging ulcers? I know the MDS has to be down-staged as this has to be the actual stage of the ulcer in the ref. period, but as far as other facility documentation? Are you all charting "Stage IV, healing" or are you charting the actual stage as it currently is? What is recommended? |
