We have been told to never down stage on documentation - but to just chart measurements, drainage, color etc until closed - then we can say it is closed stage x.  However our resident care manager for skilled came back from a wound seminar and said the "new rules" are you can down stage anywhere.  I've not heard of this yet.

What do other facilities do as far as down-staging ulcers?  I know the MDS has to be down-staged as this has to be the actual stage of the ulcer in the ref. period, but as far as other facility documentation?  Are you all charting "Stage IV, healing" or are you charting the actual stage as it currently is? What is recommended?
