It is the date the care plan is actually complete. Doesn't have to be
the meeting date. I write the care plans myself - sign the date I
completed them,
Then they are reviewed at care conferences.

To the group,
I need your help - The VB4 date on the MDS - is that the actual
date the care plan is completed (interdisciplinary team meeting and
review and update of the care plan) or can that date be before the
actual care plan date. Reading the manual it states it is the "Care Plan
completion date" so I am reading that as the date the care plan meeting
is completed with the interdisciplinary team. The MDS Coordintor is
saying no - the VB4 date does not have to match the care plan meeting
date? Thank God you guys are there to set one of us straight. Thank you
