>We are not getting informed consents signed for any medications.  How did you get started doing this?  Is this a state-related idea?  Are you doing this just for new orders for
>medications started after admission or for all of them?

We do not get signed consents because we have been told they are legally not useful. They are better than nothing but what you really need is documentation that resident SPECIFIC R&B (risks and Benefits) have been discussed.

Giving a form listing postural hypertension and falling as a risk to a quadriplegic who does not bear weight to be signed does not make much sense.  I don't think there is any shortcut around relay talking with the resident and family and documenting what was explained.
